Islington New Emergency Carers Card launched on Wednesday 11th June at Centre 404

Main points are:
• Carers can be eligible to join the scheme if either they or the person they care for has had an assessment and are eligible for services (i.e. cared-for has moderate banded needs)
• The need for the carer to join the scheme will be an outcome of either of these assessments
• The care manager will complete the statement of needs and care plan for either carer or cared-for
• Carer will complete (with the help of the care manager if necessary) the registration form and any details of the Plan of Care not covered by the care plan.
• Care Manager sends the following to Contact Islington:
• Care Plan
• Registration Form
All carers receiving an assessment or review and who are eligible should be offered the opportunity to join the scheme
